How To Access the CCAPS-Screen

You must be a member of CCMH to use the CCAPS-Screen.  All Clinical and Non-Clinical Members can use the CCAPS-Screen as a membership benefit.

The CCAPS-Screen is managed, scored, and stored in the CCAPS-Web. 

The CCAPS-Web provides CCMH members with a universal, web-based platform to enter/store/manage CCAPS-Screen data and generate Profile Reports for the CCAPS-Screen. The CCAPS-Web can be used outside of an EMR system and does not collect or retain any PHI.
